The Confidential Hazardous Incident Reporting Programme (CHIRP) in collaboration with IHMA, is pleased to provide an anonymised Safety Incident Report form

CHIRP is a charity that shares information on maritime incidents and accidents with the aim of providing lessons to improve safety to Industry.

CHIRP’s sole objective in sharing incident and near-miss experiences is to improve maritime safety. They do this in an anonymised, non-judgemental fashion to inform the industry of the ‘why?’ and ‘what?’ elements of safety learning but leaving out the ‘who?’. By doing so CHIRP is encouraging the sharing of knowledge, free from the fear that doing so could adversely affect reputational or commercial interests.
